Our team of highly skilled family medicine physicians at SLMC is committed to addressing the ongoing primary care needs of patients and their families with a focus on prevention, early detection, evaluation and treatment.

We provide comprehensive care to a diverse population of individuals and families of all ages.

Please notify our reception staff when you arrive if you are here to see the nurse for an injection such as a flu shot, allergy shot, TB test or other vaccination.

Parking spaces on site are limited and we charge a small fee. There are other large parking lots available in the area, and within walking distance.
